Iowa plans to phase out its Medicaid Integrated Health Home program by January 2026 as it launches community behavioral health clinics statewide this month.

Iowa announces program transition

Iowa Public Radio reports that Iowa Department of Health and Human Services officials plan to phase out the Medicaid-covered Integrated Health Home program by January 2026. The program coordinates care for Iowans with severe mental illnesses through a network of professionals.

According to Iowa Public Radio, the phaseout began in July 2025. Adults are expected to transition off the program by the end of September, while children are expected to transition by the end of December.

New behavioral health clinics launch

Iowa Public Radio reports that the state is launching Certified Community Behavioral Health Clinics this month as part of a new statewide behavioral health system. State officials said the clinics will offer mental health and substance use services.

An Iowa HHS spokesperson told Iowa Public Radio that, unlike the Integrated Health Home program, the new clinics are intended to serve people regardless of diagnosis or insurance status. The spokesperson said the prior program focused on care coordination for a limited group of Medicaid enrollees with mental health diagnoses.

Transition plans and provider concerns

According to Iowa Public Radio, Iowa HHS said care coordinators will contact current Integrated Health Home members, discuss options, look for possible gaps in care and develop individualized transition plans. The department also said managed care organizations will add staff to work with people leaving the program.

Iowa Public Radio reports that some providers and family members expressed concern about the timing and clarity of the transition. Providers told the outlet they were uncertain how quickly people could connect with new case managers and whether services would continue without disruption.

The Iowa HHS spokesperson told Iowa Public Radio that the department had identified significant issues with the Integrated Health Home program and had discussed its future with providers and members for more than a year. The spokesperson also cited a January settlement in a class-action case involving Medicaid-eligible children with severe emotional disturbances.
